The College of Science and Mathematics (CSM) has updated our Repeat Override Policy for all courses in our college. KSU's university-wide policy mandates that a student request permission for a third or higher attempt in any course from the college that houses it. The CSM requires that you complete a CSM Repeat form. Third repeat requests will now require completion of a student success module for consideration. We will review your form submission and either grant access to the required student success module, approve, deny, or request additional information so that we can better support you and your success.

You should ONLY use this form if you are requesting a 3rd or higher attempt of an ASTR, BIOL, CHEM, ENVS, GEOL, MATH, PHYS, or SCI course. Keep in mind that withdrawal (W or WF) count as attempts of a course.
